Gun violence represents a critical public health crisis in the United States, exacting a devastating toll on individuals, families, and entire communities. Emerging research has begun to elucidate how exposure to gun violence, across multiple tiers, converges to influence population-level health and well-being. This conceptual framework asserts that the persistent presence of gun violence in structurally disadvantaged communities engenders broad consequences for mental, physical, and behavioral health, operating as a key driver of racial and socioeconomic disparities.

Conceptual Framework

Gun violence encompasses a diverse range of intentional acts involving firearms, excluding suicides, domestic violence incidents, and mass shootings in public spaces. Rather, we focus here on community gun violence, defined as “intentional acts of gun violence between individuals typically known to one another, usually outside the home and in response to routine disputes.” Importantly, communities experiencing high rates of community gun violence also disproportionately contend with other forms of firearm-related harm, including police-involved shootings.

Our three-tiered exposure (TTE) framework illustrates how direct, secondary, and community-level exposure to gun violence coalesce within local contexts to shape population health outcomes and related inequities. Direct exposure involves being the survivor of a shooting incident, often with lasting physical and psychological trauma. Secondary exposure encompasses the indirect impact on family, friends, and violence prevention workers who know the victim. Community exposure describes those who live in neighborhoods where shootings are pervasive, witnessing incidents or merely hearing gunshots.

As social proximity to gun violence increases from community to direct exposure, the likelihood of a traumatic stress response also rises, posing greater ramifications for mental, physical, and behavioral well-being. Conversely, the prevalence of a particular exposure type within a given community increases when moving from direct to community-level exposure. This dynamic reflects the reality that in any locale, community exposure is the most common, followed by secondary and then direct exposure.

Epidemiology of Gun Violence

Gun violence remains a persistent and costly public health crisis in the United States. In 2022, more than 48,000 Americans died from gun injuries, with estimates indicating that the number of non-fatal shootings is typically more than twice the annual death toll. One calculation suggests the total economic cost of gun violence exceeds $550 billion annually.

Importantly, the burden of gun violence is not evenly distributed. Rates of gun homicides are 25 times higher in the US than other high-income countries, with the disparity reaching nearly 50 times higher among 15-24-year-olds. Structurally disadvantaged Black, Latinx, and Indigenous communities that experience concentrated poverty, residential segregation, and disinvestment disproportionately contend with elevated levels of community gun violence. In fact, neighborhoods containing less than 2% of the country’s population account for about a quarter of all gun homicides.

Psychological and Physiological Impacts

Emerging evidence demonstrates that exposure to gun violence at any tier – direct, secondary, or community – negatively impacts mental and physical health. Survivors of shootings often face lasting physical disabilities, mental health challenges such as depression and PTSD, and an increased risk of substance misuse and re-injury. Those indirectly exposed via the victimization of loved ones or community members also experience extensive psychological trauma, including feelings of betrayal, alienation, and persistent grief.

Beyond the individual, the physiological consequences of gun violence exposure reverberate throughout entire communities. Chronic stress and hypervigilance in response to pervasive gun violence impair cardiovascular, metabolic, and immune system functioning, exacerbating pre-existing conditions and generating new health problems. This allostatic load can have particularly severe consequences for children and adolescents during critical developmental stages, leading to lasting neurological changes, heightened inflammation, and even epigenetic impacts that transcend generations.

Socioeconomic Determinants

The social determinants that give rise to community gun violence are deeply intertwined with historical systems of oppression and structural inequity. Concentrated poverty, inadequate resources, racial segregation, and other manifestations of injustice converge to foster environments conducive to persistent violence. Consequently, the negative health impacts of gun violence exposure disproportionately accrue within marginalized communities, becoming a central driver of population-level disparities.

Those most impacted by gun violence often have limited resources and fewer opportunities to relocate, leading to compounded exposures over time and across generations. In this way, the intense concentration of gun violence in disadvantaged neighborhoods serves to perpetuate the very conditions that spawned the violence in the first place, resulting in a vicious cycle of trauma and poor health.

Policy Implications

Addressing the public health crisis of gun violence will require a comprehensive, evidence-based approach spanning prevention, intervention, and mitigation. Crucially, efforts must extend beyond a narrow focus on individual risk factors or criminal justice-centric solutions, tackling the upstream, systemic drivers that shape the unequal burden of gun violence exposure.

Strategies to prevent shootings and support survivors should empower community-based outreach, employ focused deterrence models, and leverage hospital-based violence intervention programs. These approaches, when adequately resourced and integrated, can interrupt cycles of retaliatory violence and connect those impacted to critical health, social, and economic services.

Simultaneously, policymakers must pursue structural reforms that address the root causes of gun violence, including economic inequality, disinvestment in public goods, and institutional racism. Interventions targeting these social determinants – such as affordable housing, living-wage jobs, and restorative justice initiatives – hold promise for reducing the uneven distribution of gun violence and its health consequences.

Future Directions

Effectively addressing the population health impacts of gun violence will require enhanced data collection and research efforts. Robust, longitudinal surveillance of fatal and non-fatal shootings – disaggregated by intent, location, and demographic characteristics – is essential for illuminating the scope and dynamics of the crisis. Pairing this quantitative data with qualitative, community-engaged scholarship can elucidate the lived experiences and protective factors present in neighborhoods most impacted by violence.

Moving forward, an interdisciplinary, public health-centered approach que​​stions traditional hierarchies of evidence and centers the perspectives of those most affected. Collaboration among researchers, policymakers, practitioners, and community members will be crucial in developing holistic, equity-focused solutions that dismantle the complex, intersecting systems perpetuating gun violence and its detrimental health consequences.
